Non-urgent advice: Please note
As a patient you are registered with the Practice not a particular Doctor as was previously the case. All efforts will be made for you to see the Doctor of your choice but this may not always be possible.
If you wish to register with our Practice please call in and complete a registration form.
We will require photographic ID & proof of address with completion of form HS200.
For foreign national patients photographic ID, proof of address, passport & work visa will be required with completion of form HS22X.
Each new patient will be offered a health check at their first consultation. This will provide a general check-over, with blood pressure, weight and risk-factor analysis. It also allows computerization of records.
Patients will be given an appointment time at registration to see our nurse for new patient medical.
